The Centers for Medicare and Medicaid Services recognizes The Birches Health Center

(Plymouth, MN) – Aug 2023 – The Birches Health Center at Trillium Woods, a LifePlan community in Plymouth, has earned five-star ratings across the board from The Centers for Medicare and Medicaid Services (CMS).

“Earning five stars in each category is a difficult achievement few retirement communities accomplish,” says Samantha Paider, Health Center Administrator at The Birches. “We deeply appreciate our associates in every department who work diligently every day to make this possible. Since opening eight years ago, The Birches at Trillium Woods has consistently been top-ranked, this year earning five stars across all CMS categories – overall, staffing, health inspection, and quality measures.

The Five-Star Quality Rating System was created to help consumers, their families, and caregivers compare retirement communities more easily. Communities awarded 5-star ratings are considered above-average quality.
